Upon completion of your Learn2Serve Utah On-Premises Alcohol Seller/Server or E.A.S.Y Off-Premises course, we will report your training records within 30 days to the Utah Department of Human Services (UDHS). Once reported, you will be able to get access your State-issued Utah Alcohol Seller/Server Card.
To access your State-issued Utah Alcohol Seller/Server Card, visit the UDHS website. You will to provide your ‘Last Name’ and ‘First Name,' as seen below. Then, click ‘Search’.
Your training record will show up after you select 'Search.' Click on your name under the ‘Server Name’ column (see below).
After selecting your name under the ‘Server Name’ column, your Server record will list the following information: Server Name, Provider Name, Trainer Name, Certification Date and Expiration Date.
After clicking the ‘Generate Certificate’ button, your State-issued Utah Alcohol Seller/Server Card will show up and you can print it out right away.
Please note that aside from State-issued card, we have our own official and State approved Utah Seller/Server Card or Certificate that we issue after completion of your online course. Our official Utah Seller/Server Card is a printable certificate at the end of your course. We do not mail this out. You can access and print this certificate at any time you may need it,
